Medicare/Insurance Claims

Dr. Teska is a participating Medicare physician and a provider for most major managed care companies. Our office files insurance claims directly to these insurance companies as well as Medicare. Those patients with private insurance will be given an itemized statement suitable for submitting to the insurance company for reimbursement. We will be happy to assist you with any insurance questions you may have.

 

Payments

We appreciate payment at the time of each office visit. For payment we accept checks, cash, American Express, MasterCard and Visa.

 

New Patients

New patients are welcome at our practice. Please refer to the New Patient Info page for information on eye exams and for the ability to print out new patient paperwork.
